Child Care Screening Tool

We have provided a link that will take you directly to the London and Middlesex Health Unit's website where you can see an updated screening tool and questions. You do not need to print this form and bring it to the child care. The screening can be completed at home before drop-off. If you have answered YES to any of the questions in the screening form, please stay home and follow the proper procedures provided in the screening documents.
If you develop any symptoms of COVID-19 or test positive for COVID-19 on either a PCR or a Rapid Antigen Test, please follow the proper protocol laid out by the health unit.
